WHO is this for? Undergraduate students, graduate students, and alumni.
WHY would I use this form? Students and alumni can request unofficial and official transcripts through the Bridge. Use this form when you are unable to access the Bridge or unable to pay online via Visa or Mastercard. As of July 1, 2020, the charge for printed, official transcripts is $10 per copy.
WHEN do I submit this form? You should submit this form when you require an official academic transcript and are unable to access the Bridge or pay online. Keep in mind: after receiving this form, we will produce your transcript upon confirming you have submitted payment for all the copies requested, so you’ll want to ensure that your transcript has everything you require recorded. If you are waiting for a grade, grade change, program completion or another change to your transcript, wait for the change to be recorded before submitting this form.
WHERE do I submit this form? Send the completed form by email to firstname.lastname@example.org and arrange for payment with the Registrar’s Office (by email email@example.com, by phone 403-320-5700, or in-person at the Lethbridge Registrar’s office - SU140).
WHAT happens next? The Information Centre team in the Registrar’s Office will process your form. When the transcript is produced we will follow your instructions (via the form) to make sure your transcript is delivered appropriately.
