T4 INFORMATION
Good Afternoon,
The University of Lethbridge is only providing your 2003 T4 slip in an electronic format. Your T4 is currently available via the Web Information System.
Please click on this link and follow these instructions to access

¢ Log on with your ID Number and PIN
¢ Click on "Employee";
¢ Click on "Tax Forms";
¢ Click on "T4 Statement of Remuneration Paid";
¢ Tax Year - Choose 2003
¢ Employer or Institution - There may be more than one option
(i.e University of Lethbridge 1, University of Lethbridge 2, etc.).
You will need to print all T4's individually if more than one exists.

Is an electronic T4 acceptable by CCRA?
The electronic T4 that is accessed through the Web Information System
displays an approval number on the bottom right of the page. The approval
number is issued by the CCRA (Canada Customs and Revenue Agency) to
indicate that the T4 Web Form has been approved. The T4 Web Form can
be printed and submitted for tax purposes.
Will all University of Lethbridge employees receive their T4 slip
via the Web Information System?
Only individuals receiving a January 2004 UofL payroll voucher will
receive an electronic T4 slip. An individual that worked for the UofL
during 2003, but has resigned/terminated, etc. prior to January 31,
2004 will still receive a hardcopy T4 slip by mail.
Will T4A's be accessible via the Web Information System?
No. CCRA has not made T4A's available electronically. T4A's will be
sent hardcopy to your permanent address.
What period of time is the T4 slip for?
The 2003 T4 includes earnings and deductions accrued within the taxation
year (January - December 31, 2003).
Why does the Year to Date Earnings total on my pay voucher not match
Box 14 (Employment Income) on my T4?
The Year to Date earnings total on your pay voucher includes all employment income before deductions, not including the value of taxable benefits. In comparison, Box 14 includes all employment income before deductions plus the value of taxable benefits and any additional compensation that was paid during the year.
What are taxable benefits?
Taxable benefits include all employer paid Alberta Health Care premiums
and employer paid group life insurance premiums. These benefits are
reported in Box 40 (Other Information) on the T4 slip.
What do the following Boxes mean?
Box 16: (Employee's CPP Contribution) includes your Year to Date CPP
deductions.
Box 18 (Employee's EI Premiums) includes your Year to Date Employment Insurance premiums.
Box 20 (RPP Contributions) includes the total amount of pension contributions made to the UAPP or PSPP plan.
Box 22 (Income tax deducted) includes the total income tax deducted
from all taxable remuneration.
Box 44 (Union Dues) includes all deductions taken for Union dues.
Box 46 (Charitable Donations) includes the total amount deducted from earnings for donations made to a registered charity (example: United Way).
