New Health Spending Account Claim Form - 2005

Blue Cross has come up with a new form to be used for submission for Spending Accounts to alleviate previous problems associated with submission of vision claims and any future areas of concern in this regard.

This form puts the onus on the member (you) to determine what will go through your spending account as allowed by Revenue Canada (CRA). There is a box to mark if you want it checked against your core plan prior to it being processed through your Health Spending Account (non-taxable).

For glasses, contacts etc. that are NOT being purchased due to eye surgery, you need not mark the box.

For Parameds (chiropractic, massage therapy, acupuncture, etc.) you need not mark the box.

For items you are NOT SURE whether coverage is also under the CORE plan, we recommend you mark the box to ensure you receive any entitled coverage under the CORE plan first and you are not paying excessively out of the HSA if not
needed.

The new form is sent directly to Blue Cross as usual.
