One Line Submission Form for Long Service Award Recipients and Retirees

One Line Comments for Long Service Ceremony

Human Resources has supplied the following form for the submission of a one line comment for the recipients of a Long Service Award and for Retirees.  The information will be used for an electronic presentation that will run at the Long Service and Retirement Ceremony on Wednesday May 1st at 2:30 in SU 300.

While we realize that it is very difficult to limit your comments to one-line we ask that you keep it as short as possible.  If you have any questions please do not hesitate to call Ken McInnes in Human Resources, 329-2753, or e-mail him at ken.mcinnes@uleth.ca .
