• Letter of Recommendation Form Please note that a written letter must accompany this form, and that three references are required (at least two must be academic).

• Program Proposal and Endorsement Form
• Appointment of Graduate Research Assistantship (Trust Funds) Form
• Request to Take an Undergraduate Course at the Graduate Level Form
• Western Canadian Deans Graduate Studies (transfer classes) Application
