Please note that all documentation must be submitted online or recieved by the Admission's Office on or before the application deadlines.

Application Forms

Application forms required by Applicant:

• Applications to the School of Graduate Studies at the University of Lethbridge are submitted via the Graduate Online Application System.
• Three completed Letters of Reference are submitted directly by the referee via the online system.

Application forms required by Supervisor:

Supervisor completes on Graduate Online Application System:

• Program Proposal and Endorsement form.  
• Appointment of Graduate Research Assistantship (Trust Funds) form
• Letter of Support 
  • detailing proposed research, reasons for working with the applicant, financial support and expected duties. This letter should also address anything that aids the committee in making a decision regarding the application.

Other forms that are required by supervisors (if applicable) can be submitted to the SGS Office (AH115) or emailed to sgsinquiries@uleth.ca. 

• Request to Take an Undergraduate Course at the Graduate Level form
•  Western Deans' Agreement authorization form  

Post Application Forms 

Completed post application documentation can be submitted to the SGS office (AH115) or emailed to sgsinquiries@uleth.ca.

• Post-Admission Thesis Proposal form
• Post-Admission Thesis Proposal Form
• Prerequisite Waiver (for Graduate Level Courses)
• Change of Supervisory Committee Members

• Information/Application for Human or Animal Subject Research Proposals
• MA Progress and Standing Report Form
• Graduate Payroll Deduction Letter

Completion Forms
