Print out this form and take it to any branch of the Bank of Montreal in Canada.

You do not need to be a client of the Bank of Montreal, however there is a $1.50 service charge levied by the bank for this service.

Fill out the form completely. Be sure to include your U of L ID number, name, date and use the semester code to indicate towards which term you are making a payment. If you are paying for more than one semester, enter the earliest semester code.
