Student Teacher/Intern Request

Teachers requesting a PS III Intern will need to complete the doc version of this form to facilitate submission of the form for their school administrator and superintendent's signatures of approval.

Section I: General Teacher Information
Should changes occur to the grade level or subjects you're teaching in the upcoming year, please advise us and we'll update our records.
Section II: Student/Intern Teacher Request Information
Please indicate the level(s) of practicum student you are interested in. We cannot guarantee preferences or placements, however will do our best to accommodate your requests.
Elementary level placements are occasionally required as well.
PS III Usually Secondary in Fall, Elementary in Spring.
If you are requesting a PS III Intern, you will need your school administrators signature as well as will need to forward your request (including your Professional Development Proposal Summary) to your Superintendent for approval as well.
