2. Key/Card Online Authorization Form(only PIs should fill out online and submit | CCBN is a secure building. This means that the outside doors are locked outside of normal business hours. The rodent facilities are locked 24/7. Only your supervisor can grant you security clearance. Note: you must provide evidence to your supervisor that you have been granted access to the rodent facility by the Vet or her assistant prior to gaining access to these quarters. |

   Please note: The vet must provide approval for the student prior to granting access to the animal side.

   If this is a new activation, or this is a replacement card, or if your card has expired more than 1 month you must take your University ID card to the security office to have it coded. If you are requiring a continuation of an already coded card then you will not have to go to the security office. The security office is located on the 9th level of the LINC building, directly across from The Station and just before the connection to the University Centre for the Arts.)
