NOTE: Students wishing to drop a course to add an Applied Study or require Full Time Student Status, must submit their application before the advertised ADD/DROP deadline.

APPLICATIONS MUST BE RECEIVED AT THE CAREER & CO-OP SERVICES (AH154, Anderson Hall) NO LATER THAN 4 p.m. on the Deadline.

• Complete the Fieldwork Placement Form
• Registration Form
  Note: Students doing Disciplinary Credit Applied Studies must obtain their Faculty Advisor's Signature and the corresponding Department/Area Chair's Signature on their registration forms
• Complete the "Proposed Course Outline Form"
